Adequately investigating incidents facilitates learning from incidents. It gives directions what to adjust and convinces of the need to adjust , in order to manage the risks to an acceptable level.

Adequate investigation is teamwork. Your investigation team needs to know how and has to be experienced in executing unprejudiced, bias-free and methodological-based investigation.
